This agreement is entered into between
an Educational Support Group (ESG) and the Asthma and Allergy Foundation of America (AAFA).
AAFA agrees that it will provide certain benefits of affiliation to the ESG to be described by AAFA from time to time and in writing. In addition, AAFA agrees:
To provide the ESG with a certificate of affiliation with AAFA. AAFA will designate the ESG as an AAFA-Affiliated ESG as long as this agreement is in effect.
To refer patients, caregivers and other individuals who are seeking a local asthma or allergy support group to the ESG via AAFA’s website, other communications and AAFA staff.
To provide the ESG with publicity of AAFA’s choice, on AAFA websites, social media platforms, including Kids with Food Allergies (KFA), and public communications at no charge to the ESG.
From time to time, to provide the ESG with certain AAFA and/or KFA educational materials selected by AAFA and/or KFA, at no charge to the ESG.
AAFA will not be liable for the acts and omissions of the ESG, ESG members, leader or medical advisor.
The ESG agrees:
To provide AAFA with the name and contact information for the ESG Leader, Medical Advisor and a current list of at least 5 ESG members.
That the current ESG Medical Advisor will sign the medical advisor agreement and deliver a signed copy of the agreement to AAFA.
That the ESG Leader is prepared to demonstrate outreach to members via regular face-to-face meetings, teleconferences, and/or webinars.
To make a good faith effort to share AAFA/KFA resources and materials with patients and the community, as appropriate.
To participate in an annual survey of ESGs conducted by AAFA.
That only qualified professionals will present medical information to ESGs in meetings (qualified professional can meet this requirement by presenting it through any means by which all participants may hear or see), in ESG newsletters, blogs, social media or other communications platforms.
That no ESG leader or member may give any medical advice at any time.
To hold AAFA harmless for the acts and omissions of the ESG, its leader, members and medical advisor.
AAFA and/or the ESG each have the right to end this agreement without cause, notice or right to appeal by sending a letter of disaffiliation to the other party at:

ESG: at the last known ESG Leader at his/her last known mailing address.
